Child Care Applicant Checklist

The Child Care Assistance Program offers assistance with the costs of child care based on the size of your family and your family's income. The assistance payment will be issued directly to the child care provider. You may be responsible for a co-payment based on your need determination. Please visit or contact your regional Child Care Assistance Office to determine eligibility or if you have any further questions.

The following items are needed to establish eligibility for child care assistance:

  • Current proof of earned and unearned income (including child support) for applicant and biological parent living in the household, if applicable
  • School schedule, if applicable, for applicant and biological parent living in the household (if applicable)
  • Verification of birth for all applicant children
  • Proof of residency
  • Name, address and phone number of the child care provider selected (for new registered home providers, the provider’s social security card and picture ID are needed)
