Protective Services Criminal Records Check (CRC) Forms and Instructions for Prospective Foster/Adoptive Parents

  • Out-of-State Adam Walsh/NM Abuse & Neglect Checks
    Please fill out the abuse & neglect form and have your applicant(s) sign it in front of a notary. Should any part of the form be incomplete the form will be sent back to be completed.

CYFD Protective Services Background Check Forms and Instructions for Prospective Foster/Adoptive Parents

  • CYFD Field Office Instructions and Forms
    1. First fill out the CRC Application. This application will help you register your applicant(s) to get fingerprinted. (Note: If applications are missing information, the CRC Unit office will notify the sender via e-mail and ask that the form be completed and re-submitted.)
    2. Register your applicant(s) to get fingerprinted.
      • Go to https://cogentid.3m.com/nm/index_NM.htm
      • CYFD is responsible for the $44 fingerprint registration fee.
      • Choose the appropriate ORI and Reason as indicated on the CRC Application. (Note: If the "Type of Application" on the CRC Application does not match the ORI and Reason, the applicant will have to get re-fingerprinted.)
      • Print and provide the Fingerprint Registration Receipt to your applicant(s) so they may go to a Cogent Fingerprint Site to get fingerprinted.
    3. Print the Fingerprint Submission Receipt after your applicant has been fingerprinted. To print the Fingerprint Submission Receipt, you will:
      • Go to https://cogentid.3m.com/nm/index_NM.htm.
      • Under "Applicant Use" section, click on "Print a fingerprint submission receipt". Enter the requested information (registration ID# and DOB) or (last name, first name, SSN & DOB), whichever you prefer.
      • Please ensure that the Receipt indicates that the applicant has been fingerprinted. (Note: If you see "Not Fingerprinted," please contact your applicant(s). Do not send us the CRC Application and Submission Receipt until they have been fingerprinted.)
    4. The CYFD Field Office shall then scan and e-mail the:
      • CRC Application and
      • Fingerprint Submission Receipt (not Registration Receipt)
        to CYFD.PSCriminalReco@state.nm.us
        (Note: Please scan the two documents together, save as a PDF file and send together. Should the e-mail not include both documents, the CRC Unit will notify the sender via e-mail to re-submit their request.)

Treatment Foster Care (TFC)/Child Placement Agency (CPA) Instructions and Forms

  1. First fill out the TFC CRC Application. This application will help you register your applicant(s) to get fingerprinted. (Note: If applications are missing information, the CRC Unit office will return the entire packet so that the application can be be completed and re-submitted.)
  2. Register your applicant(s) to get fingerprinted.
    • Go to https://cogentid.3m.com/nm/index_NM.htm
    • Applicants shall pay $44 fingerprinting fee directly to Cogent via credit card and/or money order.
    • Choose the appropriate ORI and Reason as indicated on the CRC Application. (Note: If the "Type of Application" on the CRC Application does not match the ORI and Reason, the applicant will have to re-pay and get re-fingerprinted.)
    • Print and provide the Fingerprint Registration Receipt to your applicant(s) so they may go to a Cogent Fingerprint Site to get fingerprinted.
  3. Print the Fingerprint Submission Receipt after your applicant has been fingerprinted. To print the Fingerprint Submission Receipt, you will:
    • Go to https://cogentid.3m.com/nm/index_NM.htm.
    • Under "Applicant Use" section, click on "Print a fingerprint submission receipt". Enter the requested information (registration ID# and DOB) or (last name, first name, SSN & DOB), whichever you prefer.

  4. Please fill out the Abuse & Neglect Form and have your applicant(s) sign it in front of a notary. (Note: Should any part of the form be incomplete, the entire packet will be sent back so that the form can be completed and re-submitted.)
  5. Mail the Background Check Packet which includes the following three documents:
    • CRC Application
    • Fingerprint Submission Receipt
    • Original notarized and signed Abuse & Neglect Form

Should you have a question about the status of a CRC application or abuse & neglect check, send an email with applicant(s) name and fingerprint date to CYFD.PSCriminalReco@state.nm.us or call (505) 827-8400.

Private Adoption Instructions and Forms

  1. First fill out the Private Adoption CRC Application. This application will help you register your applicant(s) to get fingerprinted. (Note: If applications are missing information, the CRC Unit office will return the entire packet so that the application can be be completed and re-submitted.)
    Pro-Se applications: This means you will be filing "for oneself" or "on one's own behalf."
  2. Register your applicant(s) and/or yourself to get fingerprinted.
    • Go to https://cogentid.3m.com/nm/index_NM.htm
    • Applicants shall pay $44 fingerprinting fee directly to Cogent via credit card and/or money order.
    • Choose the appropriate ORI and Reason as indicated on the CRC Application. (Note: If the "Type of Application" on the CRC Application does not match the ORI and Reason, the applicant will have to re-pay and get re-fingerprinted.)
    • Print and provide the Fingerprint Registration Receipt to your applicant(s) so they may go to a Cogent Fingerprint Site to get fingerprinted.

    Pro Se and Attorney applicants: Please include the Court Name and Address where you will be filing under "Agency Name and Agency Address."
  5. Mail the Background Check Packet which includes the following three documents:
    • CRC Application
    • Fingerprint Submission Receipt
    • Original notarized and signed Abuse & Neglect Form
